While I really enjoyed the Mass Effect series, I concur that there are a number of problems. One issue that I had noted from wandering the galaxy during the first Mass Effect is that it seemed that a lot of the planet destruction from earlier Reaper incursions seemed to be from orbital bombardment. Yet when ME3 rolls around we have a bunch of Reapers hanging around the surface of the planet for an extended period of time.

Actually, I never connected these orbital bombardments to the Reapers. I thought they were remains of wars between different organic/synthetic races in other cycles. As we learn in the series, there were several wars
not involving the Reapers: the First Contact War, the Krogan Rebellion and the Rachni Wars are just a few examples from our cycle alone. Other Cycles would most likely had wars, too. Just think about the Protheans and what Javik tells you about their history - they were conquering(!) the galaxy, annihilating races who didn't want to join their empire. And they also had a conflict with a synthetic race, similar to the Geth/Quarian War.
Sorry, but I really don't think this is a plot hole.
