I'm not saying I condone her brutality (or that I mind a twist for her character) but I could understand it.
The reason she gives for bringing Shepard back might just have been a song and dance to convince him/her, whereas she actually sees Shepard as a tool to further whatever (or whoevers) plans.
As mentioned earlier, it would probably be way too harsh a thing to drop on the players after 3 games with her.
However, I'm reminded of Hawke's mothers fate in DA2 and how that was almost too much (or too dark), well in DA in general tbh, so there is a precedent perhaps >

The only reason I even kept playing was because my wizard chick is played by Grey DeLisle, and therefore sounds like Princess Azula.
And even kinda looks like her a bit:
And for some reason that amuses me.
But all I know is I once played Diablo 2, and I played a necromancer who could blow corpses up.
The lack of corpse exploding necromancers saddens me.

Of course not, and I'm not trying to poke holes in the theory. I think it's one of the most conclusive pieces of evidence. I still can't help that the devil's advocate in me wonders just how easy it would be to have something like that happen on accident.

Yes, but my thinking on the sound should have been explain further. The Carnifex weapon was given a distinct echo sound to accompany the shot. Why would the last shot, the one that destroys the tubes and eleminates the reapers forever, be from a different gun with a shot that should have the same ambient effects as the previous shots, but doesn't. Since the final scene puts a great amount of effort into recognizing the gun, I would be dissapointed if this was just a programming mistake.
