 Hello there!

So, I have contacted EA tech support about this issue and they have directed me here to post my issue so that a developer may be able to help me. 

My Issue: 

Last night, after 40 hours of gameplay, completed the final battle and made my decision for the galaxy for my ME3 Insanity playthrough (YAY!!! ...or so I thought ). After the credits role, final cutscene shows, and it pops up with the message that Shepard is now a legend, you can continue..... etc. and I did not receive the achievement for completing the game on Insanity. I look at the accomplishments page, and it says I have only completed 26/27 missions on Insanity which to me means that for some reason it is not registering the last mission as completed since before storming TIM base, I had 25/27.

Now, I know what people are going to say that:
"Oh, you need to change your difficulty before you leave earth and cant change it at all in the game or it ruins the achievement".

Trust me people, I beat ME2 on Insanity as well as ME1 and I know you can change it once you start.

I know people might also say:
"Oh you didnt complete all the missions so you have to go back through and beat the missions you missed".

Okay, in this playthrough, I did everything and I mean every single misison in the journal and the only one I had left was the storming the final mission on earth/going back to the Citadel for the catalyst and choosing the fate of the galaxy. I do know the game is comprised of 27 missions (21 Main story + 6 N7) so like I said I did basically all the game had to offer. 

Sorry for the slight dissertation on the subject, I just have been bounced around by EA and got nowhere and all I want is credit for beating the game on Insanity (Insanity achievement) and all the missions as well (Unwavering achievement). I can provide developers proof if necessary that I did play the game on Insanity and such for the Xbox 360 if they need it.

Have you tried just restarting the Priority Earth mission? If you've beaten the game on Insanity, then the side missions you have/haven't completed don't have any bearing on that achievement, just Unwavering....if it still doesn't unlock after that, then you may have a fun little bug.

The only other thing I can think of is if you by any chance changed the difficulty after you left Earth; even if you changed it back to Insanity then this will lock you out of that achievement.

See here is the thing, as soon as I got control of Shepard in the game on earth (the very first time when he is getting released in captivity) I switched it to Insanity like you mentioned doing. Crazy glitch I know, but with what people are having trouble around these recent patches, I would not be surprised if that could be causing the issue.

And like you said about restarting the mission, I thought that too and did it but yeah no go there.

Wow, that really sucks. Hopefully someone else will show up that will have an answer for you. No offense, though, but I wouldn't hold my breath :/

No help here, but recounting my experience.

I played through insanity twice, both times with imported Shepards, before some achievements unlocked. Perhaps your game got its wires crossed as well in this respect.

I changed it immediately after I got contol on Earth and they unlocked at the end just fine. Weird.

Hmm OBakaSama you do have a point with the imported shep cause that is what I played through with on Hardcore and Insanity. That could be the issue or it could be cause of the patch or it could be something wrong in the .gpd file associated with my gamertag so who knows... way way way too much speculation and not enough answers from EA or BioWare.

Let me ask this then, when do the achievements unlock? It should unlock right after you make the decision on the galaxy correct?

Been a while since the unlocks for me as I've been through the game a few times now. Memory dictates that the last achievement unlock for me happened around the conversation with the Catalyst.

Yeah that is what I would have guessed it would happen around. Hmmmm, I may just have to playthrough a third time for it to happen maybe?

Do appreciate people commenting on this does make me feel not so alone though after reading fourms for hours I see people are having boat loads of issues!

Same here! 
It says I have completed 26/27 missions on insanity when it should be 27/27.
