Fallout 1: Dogmeat
Doesn't whine at you (except possibly in the doggy sense), has no gripes about the morality of your character, doesn't have severe psychological problems that needs to be solved, and has kick-ass critical hits. Screw Fallout 3 a million times for making him just... annoying to have around.
Neverwinter Nights 2, Mask of the Betrayer: Kaelyn the Dove
Something about her is just so great, from many perspectives. If you agree with her views, she can become a fantastic companion. And it's easy to simply think of her as a "hero-character". But if you disagree with her views, you quickly see how non-tolerant she can be. And indeed (as the game downright says), she sees things very much in black and white. A really great character. And gameplay-wise, I've always loved that you can constantly Bluff her that you agree with her views (to the glee of One-of-Many).
Fallout New Vegas, Dead Money: Dean Domino
This is much for the reason that I just came off replaying it... But Dean is one of the greatest ****s ever written in a game. Incredibly petty, downright pathetic in many ways, and pretty much a horrible person. But he pulls it off in such style that he's basically awesome anyways. "Make a move and I'll blast your ass so far through your head that it'll turn the moon cherry-pie red."

Yes!!...Alyx has always been my favourite companion from any game. I think she is the only fictional character from any medium that I actually felt an attachment to. I wanted her to be real.
Another poster mentioned Ellie from DS2. I enjoyed her "companionship" quite subsantially as well, but she pales in comparison to Alyx.
