It seems no one knows for certain which areas need to be explored for this achievement.
People are unlocking it it with a rogue player at 96%, whilst warriors are not unlocking the achievement at 97-98%. I myself cannot get any higher than 95% with my warrior player, so I have no idea on what I am doing wrong. To be honest it seems to be a very random achievement, in that it will pop up when it feels like it, but if BioWare could post the info, that would be very much appreciated.
Please could BioWare post what locations need to be discovered for the three classes.

the toolset comes with the source files from the game. there is a flag in areadata.xls which decides whether you need it for that achievement. so, yep.

anyway theres not much that isnt obvious. both floors of kaitlyns house in redcliffe, both houses in haven, orphanage and 3 alleys in denerim, its mostly obvious buildings for quests and stuff.

I am a 360 owner, I don't understand all this PC stuff.
Anyway..
Been to both floors in Kaitlyns house(although can't unlock the chest as I missed the oppotunity). Both houses in Have? I know the one right near the front and then the general store,

Seems its not a case of going to every location, but doing certain things in the locations. They should have just made it like the Mass Effect achievement where you finish a certain percentage of the quests.

well the 'quest' thing opens up a little area, i'm assuming you need to step foot there to get the achievement.

What side tracked achievement? I have the Xbox 360 mate. The only achievement I have left is the traveler achievement.

I finished a Rogue with 97% and nothing happened
I'll have you know I beat the game 6x and haven't gotten it , and I was obsurdly thorough on that 97% lvl 24 Rogue
I assume its just broken for some people , including myself and said **** it basically

Well if its a bug, BioWare should fix it. Perhaps just add a certain book in the next DLC which when the book is found, the achievement will pop.

i think there are one or two achievement bugs on xbox which bio have mentioned theyre fixing. i dont know if this is one. if you didnt do duncan's shield then you didnt travel to all locations so no achievement for you

But I thought Duncans shield was in the market of Denarim? If so I have been in the market area multiple times. Like I said, the achievement is wrong because its clearly not what areas you visit, but what you do in those areas.

its a hidden place in the warehouse in denerim. you need to enter said hidden place to get the achievement, it would appear.

Well I have a save before the mission where you get arrested. I might go back, find all the places I recently found to get it to 80%, and then if this hidden cache place works, that should take it to 81%.
So by the end I should finish with 97%. Personally I have a really bad feeling that you need to open the chest in Kaitlyns house for the top floor to register. If that is the case then its another case of Developers not explaining achievements properly.
The other bad thing is that for some reason, the traveler achievement won't unlock till you reach the end. So I have had to do the end mission multiple times only to find I still don't get the achievement.
No one has ever mentioned getting the achievement on there 2nd playthrough halfway through the game when they discovered somewhere new. By that I mean all the rest of the stats record in the game even if you switch the game off and reload a save(romance achievement and ending achievements).

Those .xls are super helpful for people like me who dont own Microsoft Office. 
I'm kinda confused why so many people assume you have Microsoft Office if you have a PC.
Maybe I should start sending around .odt's and complain if people havent installed Open Office.
