I don't how many people here are familiar with the Skulls in the Halo series of video games. Basically, you you turn on a Skull, and it makes the game harder in a unique way; and that's in addition to the usual 4 levels of difficulty (Easy, Normal, Heroic, and Legendary). The greatest challenge in the halo games is to play the campaign mode on "LASO" (Legendary/All Skulls On) in which you play on the highest difficulty with all 13 skulls activated at once.
I've heard many complaints that ME3's Insanity mode is too easy compared to previous entries in the series; so I thought might be a good idea to add a similar feature to the campaign.
The way it would work, is in the single-player menu, you would have a new option under Gameplay; "Difficulty Modifiers", which augment your opposition in a number of ways;
Trooper Shields - Enable this feature to give lesser enemies (Husks, Cannibals, Geth Troopers, Assault Troopers) additional defenses in the form of Shields, Armor, or Barriers.
Shield Regeneration - Enemy leaders (Centurians, Marauders, Rocket Troopers) can regenerate shields twice as fast if this is enabled. Disable, enemy shields regenerate as normal.
Health Benefits - Enable to give all enemies double health, as well as mild health regeneration.
Reinforcements - Twice as many enemies appear for each battle if enabled.
Power Resistance - When enabled, all enemies have a chance at resisting a power cast by the player or squadmates, reducing it's effectiveness by 50% (if this occurs, "Resisted" flashes briefly over the enemy's name). Higher-level boss enemies (Banshees, Atlas, and Primes) also gain a small chance to ignore a powers effects completely (if this occurs, "Immune" flashes briefly over the enemy's name).
Famine - Enable to remove thermal clips; all weapons must cool down after expending their magazine, but have unlimited ammo (similar to the Particle Rifle).
These are just a few ideas. Post your thoughts.
