I've intalled the Storm of Zahir and started the game (Official Campaign) all over again with a warlock, and when i get into the Flagon (Quest to show the shard to Duncan) I cant get out as whenever I use the door to get to the city, the game showes an error message and the game shuts down, have anyone faced this problem or can anyone help me with a solution, .....

My game was 1.20.1541 and I've patched it to 1.21.1549, and if I load the save game and try to go through the door, the same thing happens.
Do I have to start the game all over AGAIN .

You haven't applied the last 5 patches. There is a Patching Information sticky by Banshe that explains how and gives addresses for all the files you need. Try that, then start from an earlier save, before you restart the game.
