i made a deal with flemeth, got her grimoire and a 'robe of possession' for morrigan without killing flemeth. morrigan is grateful for the grimoire, does not even mention flemeth though and then .... nothing. it makes no difference other than some more background information about the relation between them.
you can have a chat about alternative ways to protect morrigan against flemeth, which might be the same as one which you can have before you go to flemeth anyway. when you make the choice for a spell then morrigan starts to reply how she need the real grimoire for that .. err didn't i gave you that one??? :(
anyway at the end morrigan(-flemeth?) proposed the ritual again and it's basically the same ending with or without grimoire. the only difference is some approval bonus for morrigan, got no mention of flemeth herself in the epilogue but i have my suspicions about the relation between the two of them anyway :P

And if you kill Flemeth....does she stay dead?

When I talked to Morrigan, she suggested Flemeth's death might not be permanent.  To me, that's pretty much a guarantee that Flemeth is coming back at some point in the DA series.

Codex says the same thing.  Something along the lines of "Killed by the Grey Warden . . . at least so it seems."

I thought if you left her alive and
didn't do the ritual, one of the epilogues has her killing Morrigan.
But this is second hand knowledge, which isn't the most reliable, so
someone else should confirm. I usually kill her because she's going to
kill someone else to extend her life, and at least this way I give
Morrigan a fighting chance (however questionable Morrigan is, I don't
think anyone deserves possession).

I've asked about this on other threads.  Several people have saod the ending is there, but I have yet to encounter a poster who's personally seen it.  I think people assume it's there, and take it as confirmation when other people assume the same thing . . .

(If anyone's seen it for real, please chime in!)

At present, I still believe there is no definitive evidence in any of your paths that Morrigan's tale about possession is correct.  It could be right, or it could be a mistake, or it could be a lie to get Flemeth's grimoire.

No, nothing happens. Ammo for DA2 I imagine.

Personally I kill her everytime. Who could resist the chance to fight that huge thing?
