1) If the plot revolved around Cerberus. Miranda wouldn't have been able to get close to an Alliance ship during the prlouge.
It was fanservice, but fanservice isn't a bad thing when literally 89% of the fanbase like those two characters.
2) No they couldn't. The beggining was rushed enough. Having Miranda there wouldn't make sense, and having her leave Cerberus and joing the Alliance would have been badly done... THERE WAS NO TRIAL FFS! xD
3) You really need to go back to those two games, because ME3 had the most banter, the most party interaction and the most dialogue between squadmates.
ME1 was too systematic and ME2 was too spread out.
4) Everyone can die on the Suicide Mission. If no one could, it would be boring.
5) Everybody has a fair chance of survival. The only one that gets screwed is Mordin if you don't have him on escort duty.

Really though, I don't understand some people's fascination with killing characters. I'm not very fond of Miranda, but I made sure I got her through both ME2 and ME3 alive anyway. I don't even kill Jacob.

No. Most people agree that her role in ME3 was rather lackluster.
According to your post it seems Miranda and Asari are there for sex appeal and their fans are a bunch of brainless perverts. at least that's how I get it, never knew I liked Asari only for their sexy blue bodies!
Unlike you about Asari, I understand that people love Quarians not just because of their big butts and hips. same goes for Miranda, I don't like her, but at least I know that her fans love her for her personality and not for her oversexualised ass. so don't make false assumptions.
