Melee: Specific Mechanics

So I was hoping someone in the know might be able to shed light on something I've been thinking about a lot.

I am a melee "fanatic" of sorts, although not as krogan, batarian or geth for that matter. I've been trying to find a general rule of sorts for how melee works, but without concrete data it's hard to say.

I'm confused as to what criteria are necessary for an enemy to stumble. I understand that normally heavy melee will stagger an enemy (human-size in this case), but when will quick melee do so? I've found that it sometimes staggers enemies if they're in the middle of an attack, sometimes not. Sometimes their melee attacks will stagger me in the middle of an attack, sometimes not. I presume the class you're playing as makes a difference too?

Also, sometimes enemies (taking geth or marauders for this) will attempt to rifle-bludgeon me when I'm in range, sometimes not. Sometimes they can anticipate my attack, but sometimes not. Is there a hard-and-fast algorithm for this, or is it just luck?
