A few glitches I've encountered

So I've finally started playing through the singleplayer (been playing multiplayer all this time) and After about 27 hours of play in the singleplayer I've run into a bunch of annoying glitches. Here is a list of the glitches I have encountered so far:

-Bonus power glitch: After unlocking a few bonus powers including Slam, Fortification, Armor-Piercing Ammo and Warp Ammo I decide to choose Armor-Piercing Ammo as my bonus power. After doing a few missions with Armor-Piercing Ammo I decide I want to change it to Fortification to see how that works out. I go to the Bonus Power bench in the Normandy and suddenly all of the bonus powers I have unlocked are gone except for Warp Ammo. Fortification and Slam are just gone, even though I have already unlocked them.

-Random crashing: I own a brand new PlayStation 3 Slim 320GB console that I bought just a few months ago and I'm experiencing tons of random crashes with this game. It's not a specific place where the game crashes. The crashes just seem to happen randomly. Sometimes I can play for 3 hours non-stop with no crashes and other times I can't even play 10 minutes without the game crashing. Now I know for a fact the problem is not with my PS3 since it's a brand new console that has hardly ever been used.

-Warp bonus power glitch: When playing as a Soldier the game tells me that I have unlocked Warp as a bonus power, but when I go to the Bonus Power bench in the Normandy it is in fact Warp Ammo that was unlocked and not Warp.

-Dialogue cutting out: Remember in Mass Effect 2 on PS3 how there was this really annoying glitch where the audio would cut out during many of the dialogue scenes throughout the game and then you would miss out on what the characters were saying? Well it seems this same issue has reared its ugly head in Mass Effect 3. I have now run into three instances where this has happened. All three of them on the Citadel. While it seems to be on a much smaller scale than the glitch in Mass Effect 2 it is still just as annoying.

-Squad disappearing: Many times my squadmembers will randomly just vanish during battle and I'll be left either on my own against the enemies or I'll be left with just one squad member while the other one has just vanished. This becomes a real problem on the higher difficulties because suddenly you have one or more less squadmembers helping you out against the enemy.

-Squad automaticaly using powers even though it's been turned off in the options: I turn "squad auto power use" off in the options menu because it makes it so much easier to do Biotic Explosions and/or Tech Bursts when you (the player) are in control of what your squad is doing at all times. The problem is my squad members use their powers even when I turn auto power use off. For example Liara automatically uses Singularity, Stasis and Warp and Garrus automatically uses Overload even though I have turned auto power use off. Now when playing as a Soldier this is not a problem, but when playing as an Adept this does become a problem because it's very difficult to set up Biotic Combos when your squad is off doing their own thing.

-Blurry graphics: During some dialogue scenes the character who the camera is focussing on will be extremely blurry. A good example is some of the dialogue scenes with Primarch Victus aboard the Normandy. (There are other instances this happens but this is the first example that comes to mind).

Has anyone else experienced any of these glitches? Is Bioware/EA going to fix these problems or are we stuck with these bugs?

So I just completed the singleplayer and I've run into yet another glitch. This time it is a Trophy glitch. I completed the entire game on the INSANITY difficulty, from start to finish and I completed all of the missions in the entire game and the game did not give me the "Unwavering" Trophy. I then did some research and it seems that if you play multiplayer and the game crashes during multiplayer then your "Unwavering" Trophy progress gets set back to 0. Unfortunately for me I played some multiplayer before I had finished my singleplayer run and during one of my multiplayer matches the game crashed. So I'm guessing that is why I did not receave the "Unwavering" Trophy.

Well this is totally unacceptable. I did what was required. I want my Trophy.

I have encoutered some you have mentioned but I haven't played SP since March after completing it.  Still waiting for the EC to resume SP campaign.  Not sure if these issues were fixed in the lastest patch.

Has anyone else experienced any of these glitches? Is Bioware/EA going to fix these problems or are we stuck with these bugs?

-Bonus power glitch:
Yes I have encountered this also, the bonus power that I have unlocked disappeared.

-Random crashing:
Also this.  Usually it was the elevator black screen in going to refugee docks - glitch that was related to Aria's quest.  Think this was fixed.  Other than that the only thing that freeze for me was the mission on New Horizon (meeting with Miranda's father - experiment on indoctrination)

-Dialogue cutting out:
Yes this also, similar to ME2, the dialogue is just cut off

-Squad disappearing:
Just once I encountered this one, it was on Tuchangka.  James Vega disappeared all of the sudden.

I too have ran into these problems.

I have encountered one I  haven't seen any one else address.  Only after the first patch *if my memory serves me right*   I have ran into this.  My first playthrough I didn't have this issue.

-Game Freeze After Leveling:    After landing on Mars I decide to level up and soon after exiting the menu the game locks up.  I am having to exit the game and load my last save.    I don't have to do a 'hard restart'.  Just pressing the PS button and quiting the game does it.  When I load back up I go into the menu as if nothing was wrong.

-Blurry Faces and Objects:  Noticed when speaking to some characters or interacting with objects they are blurry.  When I return to speak to them and see if anything has changed, it hasn't.

-Stores Not Loading:  When I'm at the Citadel the stores don't always load properly.  Sometimes I get lucky and I don't have to wait to board the ship to purchase items.

-Disappearing Characters:  Engineer Adams seems to be the first one I notice to disappear when I speak to him
