Ok I only have a few Acheivments left and Heavy Hitter to get a hit of 250 on one atack.
Im not sure what I am doing wrong. I have a 2h warrior with berserker and reaver he is lvl 22.
I tried all combinations I could think of but Im not getting this dmg, I get close to 200 but that is it.
If someone could help me out with this ...
Also I have been haveing a hard time to get Liliana to warm up to me I get her aproval up high as it goes and still no romance to get that last acheivment lol
I know the dialog to pick , I use her to play with in my team and still nothing.
How do you get the Heavy Hitter on 360?

I got it killing the Hermit in the forest with my mage using Mana Clash
With Liliana you have to talk to her at certain parts of the approval bar. If not she will not be interested in you. I am betting you havnt done her quest line

I got the heavy hitter with the arrow of slaying. I'm thinking maybe to get the heavy hitter with a melee use sunder armor, weakness, and that ability where all your stamina goes into the attack for damage or some combination like that?

The Hermit is located in the Brecilian forest in the southern part.

The acorn-stealing, tree stump-dwelling kind.
You can also try using Mana Clash against Gaxkang.

Try equipping items that increase your Stamina. Don't use any abilities; instead, activate Berserk, then use Final Blow. That's about the only way I'd be able to see getting it on your current character. Otherwise, create a Rogue Archer or a Mage and use either Arrow of Slaying (works on anyone), or Mana Clash (Mage's only). As a note, I tested mana Clash on the Genlock Rogue in the first room of the Tower of Ishal. A Level 5 Mage did 144 points of damage.

It has to be unlocked by your character. The easiest way to get it is start a new character. Make a mage then invest in the spell line to get mana clash. Then hit the first mage you can find with it. There is a blood mage near the exit of Lothering. Or you can just head to the circle tower and spam mana clash on all the mages there and demons.

There is a thread that state final blow will not give heavy hitter achievement in here so i guess it's only for archers and mages.

As people have said, Mana Clash is by far the easiest way to get big hits. Without even using all my debuffs, and with a mage that didn't even maximize Magic, I managed to hit one mage for 1632 damage with Mana Clash.
It simply one-shots non-boss mages. And to be honest it one-shots boss mages as well. For maximal hilarity, use Vulnerability Hex first.

I used Arrow of Slaying to get it, against a mage, yellow one, I believe. Make sure to Lethality (think that is the one that uses cunning for damage in place of strength) and lots of cunning.
She can get bugged if her approval is too high before you do her personal quest. You can get around this by breaking up with her, then doing something to trigger the romance to start again, think a couple of the gifts will do the trick.

A Virulent Walking Bomb exploding in a big crowd did it for me, to add to the list of possibilities.

Alright... It's a moderately difficult achievement to get but not really impossible.
The best abilities for this are high end damage abilities usually found near the end of a talent tree, for example:
Final Blow- Beserker- To pull this off you have to have a full Stamina bar that goes above 250. Not impossible. It also has to actually hit too, which can be rough.
Arrow of Slaying- Archery- Throw up some good debuffs like Shatter Armor, Crippling Shot, etc (And debuffs from other characters) Go into *Aim* and then hope this sucker lands.
Critical Strike- 2 Handed Weapons- I don't see this breaking 250, but who knows? Likely possible and it costs less then Final Blow.
Virulent Walking Bomb- Spirit- Works if you have a large enough group of enemies. Probably one of the most visually satisfying kills in game. Period.
Pinpoint Striking+Punisher- Unlikely but maybe possible. PS gives crits to all strikes that hit and Punisher dishes out the most damage for DW.
And then Mana Clash-Spirit-- Buff this with Spirit Might for even bigger boom. Find the nearest enemy mage and unleash.
