Has anyone ever experience this glitch in ME2?

I was on the suicide mission in the Biotic shield with Mirianda and I died fighting husks and a scion, but as when I died I glitch up even though it said critical mission failure, but that wasn't the weird part, the weird part is when I glitch up I wasn't my black, flatop Shepard but the default Shepard! Then I had to restart from the beginning of the mission as the default Shepard! Incredibly weird!
