I have looked at a few trophy guide sites but could not find an answer to my question so I figured someone here might be able to help.

For the Insanity or Unwavering trophy, can you import a ME3 character or do you have to start a brand new game?

You can import. I did it on my second playthrough on PS3 and got both trophies.

I imported an ME3 character and got the long service medal and the insanity one. I got Unwavering by finishing the Multiplayer maps on gold though.

Importing works, yeah. Just make sure to change the difficulty as SOON as you have control of Shepard. I was too excited during my first playthrough and thought I could only change it during the next mission because there was just so much going on.

Switch difficulty through the main menu before you start a playthrough. just to make sure ;p.

I imported on Xbox and got the insanity achievement on 2nd playthrough
