It has quickly become apparent that at least a couple of the major multiplayer bugs supposedly fixed by patch 1.03 are still present after updating the game. A number of users on these forums are reporting the same thing that I have experienced. Personally, I have made sure that I have updated the game, I repaired my install, restarted Origin, and even restarted my computer. As such, I am pretty confident that the 1.03 patch has indeed failed to actually fix these bugs:
1. The Vanguard Charge Glitch - This one dates back to the freaking demo. When a Vanguard triggers a charge during the lag time between the host and the Vanguard recognizing that the Vanguard's health has dropped to 0, causing the Vanguard to simultaneously begin dying while triggering the "Charge" animation. (Apologies if I botched the technical side, but this should give you the rough idea). If you are swarmed with enemies and desperately mashing "charge," this glitch is quite easy to trigger.
After the Vanguard is revived, whether via teammate or the current wave ending, the Vanguard's movement will be noticibly buggy, especially while running. When the Vanguard attempts to take cover, he or she will end up on top of the cover. Pretty soon, the Vanguard will, as best as I can describe it, end up shooting into the sky team-rocket-style while simultaneously clipping through the bottom of the map.
From this point on, the Vanguard is completely useless, trapped outside of the map, eternally falling until the match ends. It's not even exploitable as far as I can tell - there's no concievable way to snipe on the rare occasions when you aren't falling to the planet's core or whatever.
This glitch reportedly affects all platforms. It is avoided by having the Vanguard player host. But if you don't feel like hosting, or if you want to play with multiple vanguards, then the match will be a crapshoot. It's absurd.
2. The SMG Ultralight Materials Mod Glitch - This one is a bit more straightforward. The SMG Ultralight materials simply do not work for any SMG except for the Geth Plasma SMG (presumably because it was implemented in the Resurgence DLC). Pre-patch, the weight would appear to be reduced, but the expected increase to the power cooldown bonus would neither be visible on the weapons screen nor be present in-game.
Now, the weapon screen initially reflects the expected change in cooldown bonus. But in-game, there is no effect, and returning to the weapon screen will often reveal that the cooldown bonus has reverted to its pre-ultralight materials value.

We have been waiting over 2 months for these issues to be fixed - 3 if you count the demo (dunno if the ultralight material glitch was discovered in that one, but the Vanguard? Oh yeah). We finally get promised a patch... and it doesn't work.
This is just so irritating and frustrating. It puts a massive damper on my enthusiasm for the multiplayer. Yes, I know that hosting lets you work around a number of issues, but I don't want to have to tiptoe around these awful bugs, especially this long after release.
Bioware, and acknowledgement of these issues would go a long way, because I am just so aggrivated. I got my hopes up for a much more relaxed and polished experience, only to be faked out by this freaking patch. I know that you can't fix everything with this patch, but it should be perfectly reasonable for you to fix the things that you said would be fixed.

yes, the vanguard glitch is still happening after the patch, i just played as a vanguard and fell thru the map

I didn't fell but I got this jumpy behavior where the only way to attack is using charge (but it didn't take long since I was constantly dying). It completely blew up a gold mission in wave 9 :S

Personally, I'd add
* Fixed the issue that using an ops survival pack consumable
immediately before death could have no effect. It now works as intended
to the list of supposed fixes that were not implemented. Granted, it may be a lag issue, but when Bioware states, that this behavior was fixed, in my opinion one should either die or have the survival pack used and survive, not both, even while not being the host.
