I was kicked from the EA severs twice in the span of about 2-3 hours.
I was a HF Engineer with a Mattock 8 with Scope and Piercing, and A Geth SMG with Ultralight and the Thermal Clip mods.
My Mattock never got headshots, despite the clear hits to the head. Also saw a graphical glitch with my Overload power, a large black square appeared when it was used.
After reviving, I got stuck in a strange spread-eagle/crouch pose, weapons and powers still worked, but trying to move caused jiggling but no actual movement. After being killed and revived. It seemed to return to normal for a bit.
Friend #1 (Geth) said that Friend #2 (Quarian) appeared to be an asari for a few moments
Then my character was jumping and eventually fell through the map, while my weapons and rockets wouldn't fire off, my powers still worked.
And at the extraction, it gave us a Lone survivor bonus, despite all 3 of us being alive at the end, and the 2 of them were in the circle, I was somewhere above the map, I may have been in the circle.
Edit: Almost forgot, My character reacted to a banshee, a scream was heard by all 3 of us, and we wondered if the that 'punishment' was still active
