During Nature of the Beast, quest marker never disappears from the Grand Oak, stays there permanently even after quest is completed. I sided with the Grand Oak over the Hermit.
During companions' nightmares in the Fade, attempting to talk to them after killing the enemies in their nightmare results in the companion simply repeating the original conversation from when you first enter the nightmare/before killing the demons there. Happened with both Leliana and Alistair; additionally, the priest from Leliana's nightmare spoke in the repeated conversation, with no body sprite present (since she had just been killed, I guess). The camera just focused on empty air during the priest's dialogue. After attempting to talk to them again, events proceeded normally. Did not happen in Wynne's nightmare.
Glitchy converations in general; sometimes audio will not play, etc. This has already been noted though. Just saying it's also happened to me.
I also got hit by the glitch that prevents you from ever getting Leliana's personal quest.
Corrupted saves glitch with overloading merchants, I'm sure that one's widely known though, it has its own topic.
When wearing Juggernaut Armor on my main character, he along with the rest of my party keep their headgear on during some cutscenes. I do not know if this is related to Juggernaut Armor specifically, but it did not happen before I obtained the armor set, and that's really the only thing I can put my finger on as a possible cause.
Also, this one hasn't happened to me, but I don't remember seeing it in topic post. Apparently, if you follow a certain path during the Bhelen/Harrowmont quests in Orzammar (from what I read, trying to play both sides for as long as possible), it is possible to create a situation where you cannot progress any further with either quest, cannot gain access to the Deep Roads, and cannot progress to the end of the game, no matter what you do.

I have had various issues with the Redcliffe mission The Attack at Midnight. On one playthrough the game locked up in the first stage and the scout never came to tell me they were attacking from the lake while on another playthrough the undead would spawn and never come to attack.
Items can be duplicated in the same way as the gold glitch involving the junk inventory section.
Characters with limited skills, such as Dog, will still receive the level up icon once they have used all their stats since they appear to still receive ability points.
Sometimes the Reaver ability Devour does not seem to grant any health even if you are standing directly on top of a corpse.
NPC players can be hit by some player spells such as Grease even if they aren't hostile, apparently you can also kill some of them this way but I have yet to try this.

The lack of support and fixes is stopping me from playing the game and will deter me from purchasing Awakening. I would really like to use Sten to his fullest potential but seeing how he only gets 1 specialization point, Oghren serves as a better character. The dexterity bug is also hindering the rogue I made.
I haven't played DA in awhile and probably won't until these bugs are addressed. There is no point in supporting Bioware by purchasing DLC/xpacs when they don't support the game.

Also, on romance, the game never registered that I broke things off with Morrigan and started things up with Leliana, it just sort of kept up with Morrigan for the majority of the game (only interactions that changed were between my character and Morrigan, and with Leliana)
Another thing that I'm not sure of being a bug or not, but on escape from Drakon Keep, I opted to escape myself, and I walked out the front door with the 2 guards. I went back to Arl Eamons keep and... it was as if I never left. The only thing that made it not seem like the game was broken was Arl Eamon was relieved I was back in 1 piece. But the 2 guards just mysteriously vanished, there was no welcoming, and well, next. Very anticlimatic... and shocking.
I had tons of glitches involving any non-linear options (character interaction sequences, etc).
I'm not holding my breath for a patch. I was told that I should go to the social forums for my technical issues when I complained directly to EA, them saying that have no knowledge of gameplay... Therefore... the... gamers... can fix my technical glitches... right... As disappointing as it is, I don't think I will buy another game from EA or BioWare again, too many let downs and disappointments, too many glitches, too many missed details. Seriously. and no customer service to speak of.

Tenshi, I'd be ok with what you've said, if not for the fact that it was more then 5 years in the making. If it was meant to be a PC game 1st (which I understand, most games like this are PC), then there is no excuse for needing a patch for the PC this soon after launch.
The length of development doesn't really invalidate what I pointed out, though. Tales of Vesperia was also 4-5 years in development as I recall, as was Star Ocean: The Last Hope.
However, I don't know where the idea that this game and many others are "PC first" comes from. That isn't really true, not nowadays. Bioware itself is PC focused, of course, as are certain other companies such as Valve. However, companies know that the installed user base of players is about 4 to 1 consoles to PCs, and that's not even considering that most PCs are not capable of playing games of the past couple years, so they cannot possibly attempt a multimillion dollar game only on PC. It would be financial suicide. In the case of DA: O, Edge of Reality handles the console version and works with Bioware to do them. Similarly, consider an example such as Doom 3 for PC and Xbox; Vicarious Visions worked with id to do the console version.
Mentioning those games too... I didn't have a single bug throughout the entire game in Star Oceans: Till the End of Time... so why is every other interaction in Dragon Age so buggy???
This IS the most buggy glitched out game I have ever, ever ever played. And my brother told me Mass Effect was pretty much the same story. Why are we BioWare loyal when their games, though well conceived, are so poorly executed? It's a shame a reliable company isn't releasing them instead so we can have fewer game breaking glitches. Not to mention someone that isn't focused on these... xbox achievements... what are we? 12? And who cares anyway? I want to play the game for the game! not pursue bizarre achievements except to let me know how many endings or beginnings I can strive to view, not how many bloody flags I have to go galavanting off across the world to collect like in Assassin's Creed... Who made this crap anyway... /sigh

I still say the Warden Tower Shield does not exist because it is the EXACT same thing as Duncan's Shield including bonuses and I know the BW doesnt make name changed items for the sake of making them.
Recolors with diff. bonuses on the other hand get old fast for mages.
EDIT: I would like to mention that many of the problems listed in the OP are from memory leaks from playing for extended periods of time.
EDIT 2: Another possible bug would be in getting Shaperate's Blessing (all forms), Warden Tower Shield, and Fade Wall.
I dont believe that anything in the game was meant to be a random drop or stupid insanely hard to get as you can buy most of the top level stuff in the game and get the rest from side quests.
If you meant the tower sheild should not exist then i guess you are probably right but if you meant it does not exist at all, then you are wrong. It does exist and drops from sophia. It's a low drop rate but she does drop it. I've gotten it from her twice or thrice already during my playthroughs. Thing is you have to keep reloading and killing her till you get it.

"- Shale not appearing at the coronation at the end. (She appears at the end in both the PS3 and PC versions but not the 360 for some reason.)
(Example of Shale appearing the end in the PC version: www.youtube.com/watch?v=V1vt40PAkd4)
- The armor from warden's keep doesn't give the bonus for using the whole set."
Shale has never been in the coronation ceremony at the end of all 4 of my playthroughs, so that combined with other's testimony obviously tells me its a bug. As for the Warden Commander armor, I did notice on my last playthrough that the armor set didn't have a bonus stat. Not as big a deal as the last in my opinion, but still sad.
raef90, thankfully you brought up my most recent, and biggest complaint:
"I have had almost all the bugs listed on here, also when I beat the game I opted to marry the queen for the alternative, but I got to listen to Anora, but she disappeared completely, I got to watch instead a talking chair and a close up on a curtain and beam for a few minutes. Then when the dialogue ended (anora speaking the entire time) all I see is Alistair and myself, but not one acknowledgement to my being the prince-consort and her being the queen. The codex also said she was deposed and Alistair was made king, even though that was not the selected options at all."
In addition, I watch the slideshow part of it, and all it mentions is Queen Anora ruling alone and making awesome trade agreements. Total failure of coding! I'm running through the epilogue again to see if anything is corrected with new dialogue choices, but I'm not holding my breath. I generally plan out my characters and play through the game as if I'm writing a script, creating a story, so these types of bugs can be a real buzzkill for me.
If anyone has any ideas on how to work around these bugs for the correct endings or whatnot, let me know.

Have anyone read anything about upcoming bug fixing patches?
The last we've heard about Xbox bugs being addressed is community manager Victor saying that he's taken a bug list to the developers. Hopefully it's the one at the beginning of this thread as it's the most comprehensive and professionally presented.
There has been no timeline given for any fixes. It took 4 months to fix acheivements. If I had to guess, the absolute soonest we'll see gameplay bugs fixed is when Awakening comes out.

It's more of an annoyance really because it's certainly not permanent. You can always travel around the map and come back or load a save and the items will change levels again. Keep doing that until you get the level you want. Also, by the end of the game it's not really an issue because pretty much everything scales either to Silverite or Dragonbone.
Either way, I'd imagine you'd want a definitive answer from the devs on this before removing it from your list. But at times (rarely I admit), I've found it helpful to level down some things.

It's been mentioned by players of the PC(&Ps2?) version that you get xp for donating items in the ally chest. I currently do not get this bonus when I play with the xbox version.

I can confirm that when I put runes into the ally's chest I did not get any XP. I am not sure about the other items you can place into the chest.
