It probably happens in Redcliffe most often because people use the glitches to make money and sell tons of an item.

Cannot talk to Vaughan during the rescue the queen quest. Was able to trigger the conversation during my 1st (human rouge) run but not during the 2nd (human mage) and 3rd (elf mage)

The arrow is highlighted but I can not access the second tier menu.
It is most annoying as you can not gain access to your poisons, especially if you have made several different types of traps.

One glitch that REALLY pisses me off is that if you're a human noble male and *spoilers*
you get Anora to agree to marry you, then have Alistair kill Loghain, the ending gets all screwed up. Anora doesn't mention the fact that you're married at the very end of the landsmeet, Alistair acts like he's king, and at the post-corronation at the end, Anora will speak through Alistair's body and then Anora will completely dissapear.

The amount of bugs in this game is ridiculous and frankly, bioware should be embarassed. Did they test the game AT ALL before they sent it out?
