This is the second time this has happened to me.
Never got my operation Goliath pack either.
No Commendation Pack after promotion 6 characters

Same problem. I had this before and contacted EA using the live chat option in Origin. They then fixed it for me. I'm chatting with EA right now to fix it again.
Update: Had to give them my email adress and they gave me a commendation pack.

Same problem, didn't get my pack. Contacted EA support and they fixed it quickly.
Opened my pack and found: Eagle III

EA support had to add my commendation pack as well.
