I just went back and played the opening with my Femshep with Ash alive and all I got was the image of something near the elevator. Below screenshot isn't from my game, but this is what I saw too:

To me that something highly resembles Major Alenko in his original blue aromor. Would be nice if someone flycamed it...But as it is now - looks like both models are persented in cut, and are visible to some players (while for others - not). Only guess is that it may somehow depend on where VS died (bomb\\AA tower), but that needs check. Does not look like any IT stuff, really, more like glitch. As "ghost for Shep" should look more like ME1 versions of VSs anyway. And also not much like easter egg - not everyone can see them.

I don't have proper save, but I'll check my FemShep\\Kaidan alive version when I replay....IF I make myself to replay before EC hits..

EDIT: This means though that whoever you killed on Virmire still gets a bigger cameo than Morinth. <_<

If this really is a programming error, which I guess is possible if Ash's model is already pre-loaded there in case she was alive in the cut scene, and the glitch was placed perfectly in a hard to see but visible area for max creepiness, this really is the ghost in the machine.
