Recently during some of my Multiplayer sessions I have had the Xbox
'Disc Unreadable' Error popup. I can not confirm whether or not this
Error pops up during Campaign. As I have not touced the Campaign since I
finished my Insanity Run. What I can confirm is that the Disc is in
near mint condition. No visible scratches, cracks, stains, marks, or
finger prints. I do not keep the Discs in the orignal case. I use cases
from a pack of blank CDs I bought a few years back. I have both Disc
One, and Disc Two installed onto the Hard Drive as well. This Error has
shown up on both Discs, both in similar condition.
Does anyone know if this is something caused by installing the game on the Hard Drive, or DLC? I
would imagine the first DLC Pack is the cause. I did not see the Error
until I installed it a few weeks ago. I had no issues prior to
downloading them. I suppose it could be the Xbox 360. However no other
game, even ones in ' 'worn' condition have given me this Error. Any help in figuring this out would be greatly appreciated.

This happened to me a couple days back when I was walking around the Presidium Commons. Same situation; clean discs and installed to HDD. That was the first and only time that's happened to me so far. I figured it was my old Xbox 360. It had already been through an "Open Tray" phase in the past, so I figured since I had been playing it a lot recently it was going back to its old habits.

I cleared the Cache on the Xbox 360. Yet the Error appeared again. Granted it is not very often, but it is still annoying.

It seems that it is appearing more often now. I do not understand why this keeps happening. My copy of Call of Duty: Black Ops has cracks(Thanks to faulty original case) starting in the inner ring, and extend outward. Yet it works as if there is no damage because it is on the Hard Drive. This error is making the game almost unplayable. I am seriously considering taking my copy back to Gamestop.

I'm having the same problems too, it's pretty strange. None of my other games are doing it, and I never have the disc problem while playing singleplayer.
It seems that if you got the error while joining a game, and you try to rejoin the game, you'll get the error again. Either that, or one of the maps is causing the problem. It seems like I consistently get the error whenever I try to play on Firebase Hydra.

I noticed this after the patch to be honest. Happened to me before launching a campaign mission.
Hasn't done it since, but I don't understand why the discs suddenly have this error :/

I isolated my problem to the Firebase Hydra map. It just wouldn't load, no matter what. So I reinstalled the DLC and now it's working.
If the error seems to happen while trying to load a particular map for you guys, and it's a DLC map, try reinstalling the DLC.

Called Microsoft, spoke to their Hardware Department. Nothing was wrong with my System from what they could tell. Spoke to EA. Only thing they could do really was have me open a port. Though connectivity was never an issue. Both were very helpful. However the issue still persists. My first game attempt after just turning the system on gave me the error. I think it is definitely something on Bioware's side.
