Multiplayer, Bronze, Firebase Hydra, Geth, wave 10, maybe not specific to pc as all players experienced.
On the objective: retrieve the canister, the player carrying the canister was killed, the canister fell through floor to the sub-floor making it irretrievable. The player was traversing the walkway on the western side of the map and was killed near one of the ramps down to the lower level.
Here is where the package ended up.
