I don't know if anybody has commented on this in the past and I haven't found anything with a simple search. There are thousands of threads some of which are hundreds of pages long.
At the end of the second dream sequence, when the transition ends, there is a gasp, and it cuts to Shepard. She's fully dressed (not that it's surprising she's sleeping in her uniform these days), and she's either sitting or standing. There isn't any animation of her sitting up, she's allready upright at the end of the transition. She then looks down at a blank data terminal that shes holding in her hand, which she then sets down and it beeps like its being turned off.
This is the dream sequence that occurs immidiately after the Tuchanka/genophage mission, and right before you get the Citadel mission with the Cerberus attack. The dream sequence is filled with whispers from Mordin or Wrex and whoever you left to die on Virmire.
My first thought was that the tablet was supposed to be a portrait of the person lost at Virmire, which would fit in with conversation with Liara afterwards and the whispers in the dream, but that it was bugged and left dark. The tablet is shaped like a communications terminal and not a portrait, but you get the same thing with the portriat that Cortez places at the memorial on the Citadel (Cortez is holding a communications type device, but when he sets it down it's left as a portrait).
However, given all the conspiracy theories floating around here, it made me wonder if its not something more sinister.
　
　
　
And speaking of odd things, has anyone noticed that the circular platform at the end where you confront TIM has the same design as every door control in the game?
