Liaras mother/father not appearing on the citadel at all. I spoke to her in me2 and "swept" each floor of the citadel after each mission.

Liara treating me as a friend until late in the game despite romancing her in me1 and lotsb (i this is an import bug).

Edit - I have just loaded a save at the citadel (just before chronos attack) to check (as some have commented) and 
Aethyta is still not there.....

-can't hook up Gabby and Ken when Kaidan is the Virmire Survivor.

-when a romanced Kelly gets hugged, both her and Shepard end up hugging air instead of each other.

-Shepard looking at Glyh, after asking about it, when talking to Liara for the first time.

-somehow on the Tuchanka mission where you need to save Victus nobody spawns at the end.

-the system where you help Jacob and the ex-Cerberus scientist will be flashing, despite all missions in that system being complete.

The Ashley romance during the date scene. Ashley's dialogue gets cut off and the following kiss looks more like Shepard's trying to suck out her eye.

Also, in the Udina coup, there's a scene after you shoot Udina and Bailey arrives to take the councilors away where the screen just shows an image of the rail behind where Udina was shot, but no Udina... I get the impression that it was supposed to be the VS (if they survive) taking a last look at Udina's corpse before leaving.... I don't know. It just breaks up the drama of the scene so spectacularly (not in a good way)

I also suffer from the bug where you get stuck in the normandy cockpit while trying to talk to edi or joker. It usually only happens when im right behind joker and i try to talk to either of them. If i stand behind edi, i usually wont get stuck.

Also, and i know this pertains to the ending sequence but hear me out, the bug where even if i use Javik rarely during the game, he still somehow ends up in the final movie sequence coming out of the Normandy with joker. To the best of my knowledge, only the characters you use most are supposed to come out of the normandy with Joker, in my case Garrus and Liara. I really don't want to see Javik there so if you could please fix that bug i would greatly appreciate it.

Liara not recognizing that we decided to be together during LotSB. And she says its been 2 years and asks if I want be with her.....well Liara we got back together during LotSB, remember? It hasn't been 2 years.....chick is losing her mind

Not sure if it's a bug, but the super expensive N7 weapons from the Spectre terminal seem to be the most useless weapons, (in ME1 they were the best and worth their price), which is before you even add in the huge weight that hampers your powers.

When talking to Liara's mother/father, the entire conversation was bugged to where everyone, even Shepard, were talking like Shep and Liara were in a relationship, when I had never at any point in the series even shown interest in her.

The fish float out the tank when I'm walking around. Don't know if it's just mine nuts kinda funny, also I'm the Ashley romance scene. Dialogue is cut off and jumps around often.

Ashley has major issues
-her eyes in the hospital go all weird, like they roll back into her head.
-after the Cerberus coup attempt, Ashley and sheps conversation. They don't look at each other and Ashley is talking sometimes without the camera in her spot and then she jumps around while talking,
Shepard looks like he's sucking her eyeball out in the citadel date, dialogue also jumps and gets cut off and shepard's eye don't sit right on several occasions in the game. No human could do some of the movements he does.

Journals don't work properly, find that they have little mission details.
When Shepard picks the rock up, when he passes it to tali. It Disappears, also find her helmet light doesn't light up sometimes when she talks in a cutscenes on Rannoch.

Shepard's head keeps turning left and right in his first convention with Liara after you ask about glyph. I fell through the floor a few times in single player missions. (different playthroughs).

A few bugs regarding Tali:
- When doing the mission involving Jacob and the Ex-Cerberus scientists, Jacob will acknowledge Tali's presence. However, when Tali replies she is invisible. Stuff like this happens throughout the game.
- Handing her an invisible rock on Rannoch.
- Prior to fighting the reaper on Rannoch, If you romanced Tali she will say "I love you" to Shepard. However, yet again she is invisible.
- Romance scene, Shepard removes an invisible mask. That one was particually grating and immersion breaking.

There is also shockingly bad bug where if you romanced Tali yet sided with the Geth on Rannoch. She'll still appear for the romance scene prior Cronos Station, literally coming back from the dead. I can't believe you guys missed this =P

There's also Ken+Gabby bug with the VS status, but that's an obvious one.

Oh, and when talking to Liara for the first time on the Normandy. Both will look sideways at Glyph whilst having their conversation, which looks more then a little awkward and weird.

There are probably a ton more, but these are the only ones I could think off.

I think we can all agree they should improve the Journal. Because as it stands now, it's completely useless - doesn't give you information where the quest giver is, doesn't tell you where to search, and doesn't track quest progress (or, for some quests, only partial information is given, like only the quest giver's location).

Some Shepards have eye related bugs. Bug during conversations with Liara looking at Gylph as pointed out is very common.

-Shepard and Liara's spinning head when talking about Glyph in Liara's cabin.

-Getting stuck in the cockpit while talking to joker.

-Shepard giving Tali and invisible rock on Rannoch.

-Seeing Liara's face in the death flashback and not Garrus (I don't know if this counts as a bug, but it sure is immersion breaking).

-The badly set out Journal. I don't really want more information on quests but just separating out N7 quests/Side quests/Priority missions would be nice (again, not sure if it's counted as a bug).

-Shepard's hand moving all over the place when using her earpiece.

-Thane's feet sticking out of the end of the hospital blanket during his death scene.

-Gabby and Ken losing their dialogue if Ash isn't on the Normandy.

-The citadel date with Garrus has some terrible graphic glitches.

Things I've noticed:

- During the scene with Wrex on the Citadel, both FemShep and BroShep alike will move their mouths to synch with Wrex's playbacked recording. It's a recording and Shep is listening to it - they shouldn't be moving their mouths. It looks like they're talking to Wrex, echoing the very damning evidence they can later imply is fake.

- Game crashing randomly during load screens on disc 2 (xbox360). The disc is still in pristine condition and this happened even during the very first use of the disc. Seems to happen during any load time, not any one in particular. Though only during the campaign - playing multiplayer on disc 2 never crashes.

- Graphical issues during Garrus' date on the Citadel. There's missing textures behind Garrus in several shots, ranging from somewhat minor to major graphical glitches.

- Missing Ken&Gabby dialogue if Kaidan is the Virmire Survivor.

- FemShep's hand keeps randomly teleporting around and often clipping into her face when using the radio (many radio scenes). This is especially immersion breaking during the paragon choice that resolves the geth/quarian conflict.
- Liara's romance import from LotSB is glitched and she will refer to not having dated for two years no matter what.

- Shepard gives Tali an invisible rock on Rannoch even though the rock was modelled just seconds before.

- Ex-Cerberus Scientists mission has squadmates disappearing in dialogue with Jacob.

- Liara's early dialogue that involves Glyph moving about has both Shepard and Liara lacks a flag for them to reset their faces to look at each other, leading to them looking away while talking. (for ultimate hilarious effect, run the investigate option that causes the glitch to happen twice - the second time it glitches even harder =))

Heavy weapons sometimes disappear when you put them down, or if you die after having unequipped them.

Haven't read the entire thread yet but this is what bothers me:

Liara does not remember the conversation aboard the Normandy in Lair of the Shadow Broker. If you start a new relationship after romancing Liara in ME1 she accepts this in LotSB. In ME3, however, she seems surprised that you hooked up with someone else.

Ken and Gabby's conversations don't trigger if Ashley died in ME1.

Shepard uses the term Crucible before Hackett calls it that. One example is in Kasumi's sidequest.

Joker says you hung up on the council in ME1 even if you didn't.

In the last Rannoch mission Tali seems amazed to be walking on her homeworld even if you took her to Rannoch in one of the sidequests. She acts as if it's her first time there even if it isn't.

Edit: To those wo say Liara's father never shows up, she's not on the minimap so you have to find her the old fashioned way. It took me three playthroughs to discover that she was behind the bar.

Oh yeah, speaking of weapons...

- The Chakram Launcher can sometimes lead to being unable to pick up thermal clips. It's only happened on one of my playthroughs, yet it happened repeatedly. I think it had to do with how I was using, dropping and again picking up a heavy weapon on Menae which set it off and then it seemed to happen several times throughout the game (know for sure it happened again during Jack's mission for instance). That was the only weapon acting like this.

hey Chris ,a bug I noticed is that alot of converstions that Shep has (more so on the Normandy) if you put the camera on him/or her while speak their mouths don't move along with the dialogue and in a lot of cases Sheps mouth stays open a after the dialogue is over. also I'm not sure but you can't holster your weapons in the field but not in combat. ME3 sqaud mates have the option of removing the helmet in combat under Naritive>options> "default off". while shep does not have that option to remove the helmet.

This was explained a while ago. Apparently if the player has the option to holster weapons in combat areas, then the hardware has to load additional animations and the Xbox 360 apparently doesn't have enough RAM to handle this. No idea why they don't allow holstering on PC though.

Yeah, the Shad-Brkr thing really bothered me too.  Liara was LI so we did the deed, but early on in ME3 when we talk about "us" Liara says something like: It's been years.  Um, Liara, it's been a few months.

I wanted to drag her to the medbay for a CAT scan to see if she had early stage Alzheimer's.

Not a serious bug but in the Mars archive after you jump from the disabled tram into the one with the Cerberus assault troopers, Shepard stops in front of the control terminal and stays there until the tram docks. While standing there, the player can move the "camera" around and shoot in any direction despite Shepard being physically stuck in a pose holding his/her gun straight into the air.

It is a relatively minor bug that doesn't do too much to break realism but I doubt it was intended. Personally I would focus limited time and resources on more important things.

Not sure if it is a bug but vent child can be seen running through an open door that appears red to indicate that it is locked. This is in the beginning just before you slide down the ladder to melee husks and have the building Shepard is on shot and damaged by a nearby reaper.

Sheperd can move to places he/she shouldn't near quest giving npc's near benches on the Citadel . The one near Purgatory that needs the ancient krogan power schematics is one. A more serious one is the asari that needs the reaper code fragment on the embassy level. Shepard can not only stand on her head if he steps on the bench first, but can walk over the balcony and drop down to the lower level. The player is not intended to see the blurry shapes up close and has no way to escape other than walking through the wall and endlessly falling down as the presidium becomes smaller and smaller. The only option then is to reload a previous save.

On the Palaven's moon mission after Shepard shoots husks with a stationary turret and a brute appears in a cutscene, before he charges and knocks Shepard down, the brute's midsection is blurry and appears low resolution.

In the first N7 mission assigned on the Firebase Glacier multiplayer map, it is possible to spawn endless Cerberus soldiers. It is does not always trigger but after Cortez returns to pick Shepard up, after all of the Cerberus guys are dead, if the player exits the landing area through the lower exit and then turns around and walks back into the landing area, the Cerberus guys spawn again. This may not be a bug and even if it is, it is pretty fun and doesn't break from the narrative. It just like another wave of bad guys.

When Wrex is confronting Shepard for sabotaging the genophage cure, Wrex's gun briefly changes from a claymore to a graal spike thrower and when fired, looks and sounds like a graal. This occurs just before Bailey comes up behind Wrex to shoot him. The player might have to skip the renegade interrupt to see it.

In Priority: Mars after Eva Core exits the crashed shuttle and bashes the Virmire survivor, the player enters into slow-motion and must shoot their pistol/hand cannon at Eva before she slaps the gun from Shepard's hand and kills him/her with an omni-blade. If it is a new game plus and the player has the scorpion equipped, the projectiles do not explode in time to stop her and it is impossible to proceed without loading a save that allows you to switch to another pistol or none at all.

During the Sanctuary mission, when Shepard catches up to Miranda, she is presumably supposed to appear injured at Kai Leng's hands. The only visible change, however is that her teeth seem to be glowing red. She is limping but the glowing red teeth are confusing and detract from the drama of the scene. I am unaware if it is similar when Miranda dies as I have never allowed that to happen in my playthroughs.

During our first conversation with The Illusive Man in the Mars archive, when the "camera" shows him in his space station, there are thick, dark lines on his face. It happens every time so maybe they are meant to be wrinkles or shadows but I don't recall seeing them at any of the other times we see him.

That is all I can think of off the top of my head without touching on a few possible bugs in the ending. The indoctrination theorist have found some possible bugs. Apologies for grammar mistakes. It is late here. I appreciate the work that the Mass Effect team does.

Thessia mission cutscene when Shep standardly has a m8 avenger when I don't use assault rifles. Kinda spoilt the moment which felt rushed.
Also the whole journal just didn't cut it compared to me 1 & 2
