What are your most important Story based bugs?

1). My Shepard killed the rachni queen. When flying to the rachni mission w/Garrus & Vega, Garrus and Shep had the "we saved the queen...why would she attack us now" dialogue. Luckily the rest of the mission played out correctly.
2). My Shepard kept the Collector Base. During the first dialogue w/The Illusive Man on Mars, part of TIM's dialogue doesn't trigger and his arm starts flailing around crazily.

This happens after missions where you use a special weapon, like the targetting laser and the gun you use in the Grid.jaktuk wrote...
Not that seroious but still kinda annoying. Sometimes when choosing weapons before going on a mission all earlier equipped weapons are removed and you are left with only the predator heavy pistol (on Shepard only). If you do not notice that you have to restart the mission and repick weapons.
Not a bug. This is done by design because it would take too long for the animators to take every weapon combination into account. This was present in Mass Effect 2 as well.Headbutters inc wrote...
Thessia mission cutscene when
Shep standardly has a m8 avenger when I don't use assault rifles. Kinda
spoilt the moment which felt rushed.

I also have some minor but pervasive lipsynch / audio problems, but that might be my less than stellar hardware - ME3 is the first in the series I've had to play on my midrange laptop instead of a proper gaming PC. EDIT: Can also confirm the getting stuck in Normandy cockpit bug during conversations with EDI and especially Joker (this is on PC).

However, I would like to note that it is awkward targetting people and items to talk to/collect. In previous games it was handled very well, and there was a decent distance you could do it from, but this time it's plain irritating.

- Ken/Gabby not happening if you saved Kaidan in ME1
- Tali's romance happening even if she dies
These are the biggest immersion breakers I can think of.

2. Aria: Blue Suns sidequest as soon as I completed the sidequest, the sidequest tells me, I got the blue suns, but when I got to the war room to check my war assets, it says that I don't got the blue suns.
3. Pritority Earth when I faced a reaper (and other enimies to stop u) to launch the thanix missles and then killed the reaper. If I didn't kill the banshees, I could see their biotic barrier being played in background in a cutscene when Anderson comes and finds me.

The Liara/Glyph staring contest that's been mentioned.
During the Citadel Date with Ashley, I don't know if the animation is funky or what but Shepard always kisses her eye, not on the lips. It's awkward.
When on the bridge and after talking to EDI and when you try to walk out of the bridge, Shepard goes flying up in the air but then comes back down once you clear the area around her chair. Not a big deal, but still.
This happened to me on Tuchanka when curing the genophage but for some reason Garrus just suddenly disappeared. As in, I only had one squadmate. He wasn't available on the power wheel nor could I issue commands to him. It was like he disappeared into thin air despite me selecting him. It happened right before Wrex/Eve talk to you on the radio right about the Kalros' plan and you fight the brute and a few ravagers..

One other bug - when you're talking to two CSEC officers on the Presidium Commons after the Cerberus attack, regardless which one you support the Citadel Defense Force update always reads: "CSEC spends more time on investigating petty crimes and its efficiency against major threats is diminished" or something like that.

SP but not exactly a story-based bug, so I'm not sure if it's on the table for your meeting: I've also had squadmates getting stuck during Priority: Earth. This is just after you clear the apartment and enable a squad to advance, then you take a fairly big drop down to street level and fight your way towards some shops. Once neither of my squadmates was willing to follow me down. They eventually respawned after the next cutscene, I think when you reach the Thanix Missile trucks.

- Label missions are timed or not
- Shepard's weapons in cutscenes is completely different to what I'm wielding (seems related to whenever Shepard is in an area with enemies). aka: I'm using a Revenant, but suddenly I have an Avenger instead
- Engineering dialogue won't trigger, if in ME1 romance with Ashley
- There seems to be some discrepancy between where the gun is pointing and the reticule is
- Matriarch Aethya still refers to me as Liara's boyfriend...even if I'm in a romance with anyone else.
- Ashley doesn't seem to have visible reaction between buying chocolates or tennyson.
- Ashley and Shepard seem completely engrossed in Glyph...even during conversation. Will actually crane their necks around to watch 'im fly.
- There is a spot between EDI and Joker in which your character freezes...I shouldn't be running for the door whenever I talk to any of em.
- Audio bug during the Priority: Earth. When the final charge begins (aka: Turians start chucking grenades, Krogan start shooting husks and whatnot), for that cutscene all I hear is the music, no SFX whatsoever.
PS: As much as I rage on the forums, kudos to the tech guys for making ME3 run better than ME2. I don't know how you guys did it, but I'm able to run ME3 with all the candy vs ME2. Thank you

- Claims to be funneling data to the Batarian fleet in order to track Reapers...but mission dialogue points to Balak just targeting humans instead. Is Balak still a jerk to humans in the middle of an intergalactic war for survival? Or is he lying?

I've also seen the Invisible Rannoch Rock, the Invisible Tali Romance Mask and the Glyph Head Turn bugs. I'm on PS3.

Sound effects in prerendered cutscenes are much louder than the in-game audio. Tested this on two different PCs with different sound cards. One has 5.1 speakers, the other 2.1 stereo.
