And she does on the Citadel, in D24 waiting area.

The first time I played the thessia mission in the beginning of the mission where you have to hop on the gun turret and kill everything if you use the turret to kill all the brutes and husks the sequence that follows won't trigger. I found a work around by killing everything but one husk, getting out of the turret, and killing it with my own weapon but I thought I should mention it.
Edit: I have the Xbox version
I also experienced this bug on last playthrough, also on XBox ...

- sometimes your squadmates seem to have serious pathfinding problems during missions.

The interior doesn't really fit the exterior. Besides that, there are no lifepods visible - same for the old SR1. Also I wonder where all those crewmembers sleep ... whatever.
The Normandy's interior is incomplete and doesn't fit the hull. It's not a bug, just a design flaw.
