Orsino's death.

But still... WHYY. WHY did Orsino have to die, even if you side with him? Things are going fine and they are winning, then he just goes all emo and turns himself into a monster... and then immediately starts hurting the very people who are trying to help him, and whom he's fought so hard to protect.
Seriously, that was just so sloppily tacked in it's ridiculous. There was nothing in his character that precipitated that in any way, it was just stupid on so many levels. He was such a good guy. (yeah, he knew about and covered up the guy who killed your mom, but he did that because he was afraid it'd be more ammunition against his people if the guy was discovered.)
Gah. So freaking sad. He was such an insanely attractive and adorable and lovable little elf.
But I hear it was because some nameless idiot executive in some other department demanded another boss battle, so they decided to screw the writers in order to get it. Ugh.
Whoever did that should have a mug of boiling hot coffee dumped over their head. GRRRRRRRR. <_<

Now hold the phone there. "Insanely attractive and adorable"? I can think of a few ways to describe Orsino and that phrase isn't one of them.

I thought he was cute in a unique way. And hawt. I'd do him.

Perhaps if they were losing and he was surrounded by Templars, he might have been pushed to such an extreme.
But they weren't losing. And the only people around him were his OWN people. It just made no sense whatsoever.
Sloppy writing, rushed content.

Storywise, they were losing. If you notice, he happens to be surrounded by a series of mage corpses when he flips. Gameplay wise, you were winning, but storywise, that fight was supposed to be a pyrrhic victory - you just slaughtered all the Templars charging at you, but those same Templars just offed the mages you were defending.
But yeah, that entire segment was a result of rushed content.

No, Bioware just wanted another boss fight. It wasn't rushed, they wanted a boss fight.

Fair enough, but still, where and when he chose to do it makes no sense. He transformed and immediately began attacking his own people, without an enemy in sight.

Yes... and it was rushed.
If it wasn't rushed, they would have created a new character or story segment to have a boss fight in, or put more thought into Orsino's placement. Instead, they rushed it, and Orsino's fight was sudden, senseless and out of place.

Which is just sad, considering that you can defeat all of the Templars without a single Mage dying. And as a result, they all die from a fungal infection of the big toe which led to a coronary thrombosis, from which they fell upon the bladed portions of their staves.
And this all happened at the same time.
I'll give them points for being synchronized about it though.

Rushed and the fact that gameplay trumped story.

Which is sad because the gameplay wasn't even that good. The Orsino-Harvester fight was no where near as challenging as the original Harvester.
But I'm going to have to be careful around mages in DA:3.
DA3 Protagonist: So, why exactly do you all wear dresses?
Mages: We can't continue living after such a grievous insult. *Transforms into Harvester*
Morrigan: Nice one DA3 Protagonist.
Oops I meant random DA:3 companion, not Morrigan.

Because Bioware was being pressed by EA and was operating within certain time constraints. Thus the ending was absolute crap and made no difference who you joined forces with.
Bioware is EA. The good doctors are EA officials. All projects work within certain budget and time constraints. It is up to the development team to set the timetable. If the development thinks it can make major changes to the engine in the time frame given whose fault is that? If the development team told the bosses we can do it in that time frame then the team will be held to their timetable and budget.
