Every time I play on Firebase Jade, I get to extraction, but I dont show as extracted, giving the whole team a partial extraction.
I usualy play as a Vaguard female, so I wondered if that was the issue, but my wife plays exactly the same, and It doesnt happen to her.
I always have top score by atleast a several thousand point lead.
I havent tried with a different class, and this is the only level that this has happened on. (that I have noticed.)
Not sure if It happens on Firebase Goddess or not.
I dont really care, since the extra few XP isnt a big deal to me, but some low level people arent real thrilled with me.

the LZ is offset, or glitched, or something. if you stand in the center of the platform instead of taking cover you should get full.

Jade is the only ME3-MP map except Dagger (it does not affect the extraction zones there) that actually has more than one "floor" accessible by the player.
You can be on the bridge (extraction zone) or below it (not extraction zone), that might be the reason why the game is picky if it comes to extraction on Jade.
