This happened during the Priority Thessia cutscene in the temple. My companions were Liara and Javik, I can hear Javik talking, but I can't see him. All I see is the background of where he is supposed to be. This also happened during the Citadel II cutscene, where I can hear my companion talking, but can only see a scenery background and not the actual character. Playing on xbox 360. Anyone else had this problem? Is it my xbox or a glitch???

It's a glitch it happens in various scenes throughout the course of the game.

Ok, I don't remember this happening during my first playthrough, but I didn't have Javik as a squadmate that time :/

back when I could play ME3, before I was hit with the Black hole I had this glitch occur repeatedly during the conversation with the assassin in C-Sec headquarters, the assassin was not there (multiple playthroughs). Also when I first encountered Jacob, Garrus disappeared. but nothing compares with the Black Hole glitch. THE ENTIRE GAME DISAPPEARS!!

For myself, characters disappearing in cutscenes tend to happen when I am skipping cutscenes. If I let the cutscene run a bit then they're usually there. Oddly, the Glyph glitch can be avoided by skipping the dialogue, at least in my experience.

Another glitch that I'm noticing (Or at least I think it's a glitch...) is that every time I go to talk to Javik in the cargo area, Shepard's hair is transparent, just around the edges, but still annoying. It only happens in that room.

You are correct, that is a glitch.
