Firebase Jade extraction bugged?

When fighting in Firebase Jade, sometimes my squad gets a "Lone Survivor" completion, even though all four members were in the highlighted area and alive.
Once I was one of the three that supposedly didn't get extracted, and I was kneeling behind cover on the middle landing area, to the far left if you're looking up the stairs, along a short section of wall so I was facing the upper area. That is clearly inside the extraction zone, yet the post-game report showed that I was not extracted.
I haven't had a mission actually -fail- yet because of this, but three times now I'm absolutely certain the team should have recieved a "Full Extraction" bonus, and instead got "Lone Survivor", with one squad member seemingly chosen at random.
At first I thought it was that I was in cover, but about half an hour ago, I was the one random person who "survived", and that time, I was again behind cover, though on the large wall in the center landing, overlooking the majority of the lower level.
Actually the other time, I was again in cover, behind the short wall section on the opposite corner of the middle area from the first time... perhaps those wall sections are bugged?
Just thought I'd mention it.

Don't seek cover on the Extraction walls get off of them, I tell everyone this at 8 secs

the extraction area has 2 LEVELS (kind of a platform and a step up from it) - Do you think it is possible that lower "level" of e.z. is not counted same way as players standing on the upper level (in e.z. area circle)? Meaning if you're not at the top portion of e.z it counts as if you did not extract?.... I tried recreating it and I think succeeded on 3 and 1/2 occations (1/2 because last 3 seconds someone had to move to make space for the big-boned brute.... I'm sure the shuttle had enough space for our new pet).

This seems possible, because I know I have had some extractions complete while under cover...I think I was under cover of the upper level. Seems like a very good possibility that they forgot to allow the right vertical range. I've also had extractions fail on the main level while standing next to a wall, not crouched.

It does make me very curious about how they determine a player is in the zone. Have you ever seen a glitched character that is jumping vertically and sporadically vertically up and down? They rarely get counted in extraction, even if they are in the zone. I have seen one player with this get lucky and get counted, but they must have some vertical rule.

I suspect that the extraction zone consists of a range of x,y,z axis coordinates that are checked against the x,y,z axis coordinates of some specific point on the players hitboxes. If this point is at something other than the floor, and the player is in cover, then this point will be skewed (compressed) into a lower value on the z axis than normal, and thus may fall outside the range of values that was input into the map's extraction zone.
