If you like open world action RPGs (Bethesda style) in modern times you should really try
Boiling Point: Road to Hell
This game is pure gold belive me, and imo is much more fun than any other FPS/RPG hybrid.
You can join various factions (for example join the rebels to fight against the goverment or help the goverment to eliminate them), you can drive vehicles, tanks, boats and even fly in airplanes and helicopters. Character development is stats/skills based and is very complex too.
There were performance problems back then on its relase but the game has been patched since then, and now on modern PC it runs smoothly without any problems.

It's kind of funny I haven't played the first two, even though I love 3 and New Vegas. I might start here. I need something to wash the bad taste of ME3 out of my mouth.

I do love the Bloodlines. Funnily enough I have never tried any mods except for the fan created patches. I should probably look into the mods people have made for it, which I imagine would be in the hundreds by now.

Have you tried Alpha Protocol? Technically flawed but otherwise excellent modern day spy RPG. Should be real cheap by now.

I know. You would think there would be more variety on PC, but it's just as barren as consoles when it comes to this. We've got, Vampire the Masquerades: Bloodlines, The Fallout series, Alpha Protocol, Mass Effect, Borderlands and that seems to be it. There have to be hundreds of medieval rpgs. How could they not be sick of making that already.

I love AP to bits. In my opinion, it's one the best and most underrated games ever made. At this point I think I've got about 7 different playthroughs.

No love for Fallout 3? New Vegas is similar in it's playstyle, but does everything else so much better. I just happen to like D.C. more than the wasteland of Nevada. My only fear is that the original two would be too hardcore for me and would scare me off from finishing it. I'm going to check those out though.
