I sat behind cover and a banshee got on top of my cover. It went to execute me but because it was taller than usual, I just floated up as the Banshee lifted it's hand up. Banshees are jedi now?

I always panic when surrounded by enemies or when i'm the last one standing. Many times i've failed just because of that. Sometimes even on bronze. (yeah, i know...)

Concentrating too hard on trying to Cobra two fairly widely separated Atlases from across the map, I didn't notice the squadmate next to me being instakilled by a phantom.  Whilst I was congratulating myself on the successful missile shot, the bloodthirsty Cerberus lady decided to show me that pride comes before a fall...

Silver Reactor vs Reapers. Wave 10 objective is delivery. Some drell guy beats me to the first package barely, which is on the far end of the map from the LZ/Drop-off point. Quickest way back to the LZ was through the Reactor which me and the others kept clear. Should have been a piece of cake, but I then noticed that the drell hadn't moved for like 20 seconds at least from where he grabbed the package... I didn't have my mic on, but I think he thought we were doing a Hack mission and didn't realize the button he activated stuck a huge backpack on is back...

I rushed up to him and started shooting him in the face at point-blank to get his attention. At this time I'm assuming a lightbulb went off and he began to move towards the reactor. A couple of brutes and a banshee were just rushing in, so I popped a rocket and cleared the way. The other two were still 'holding the line' as I turned back around to check the drell's progress...and then noticed he got a brain-fart and took the scenic route that runs along the longest route at the edge of the map and right smack into a banshee tag-team...

Needless to say, we couldn't reach him in time, which resulted in a total wipe...

Just had a Gold / Geth one.

Medigel then missile 3 pyros and run straight into a Prime and get poked to death.

Somewhat similar, saw a bad situation, Brute and Banshee coral the teammates to corner which seemed like a perfect opportunity to use a missile, while the missile is getting readied suddenly the two big enemies start to move opposite directions. A bit of greed hits me and i figure that "If I aim exactly between them the missile will kill them both"... and lo...

Almost always when I try to activate my Ops Survival Pack, I'll miss the button and switch to Missile Launcher instead (or try to use Medi Gel in vain). Most of the time it doesn't get me killed, but when it does, I bet I look like a crazy Muslim trying to blow himself up.
