Like falling through maps, this was built into the game (unintentionally), and no outside manipulation is needed. All it takes is three button presses and a high enough latency to pull off. It's random.
There is no hacking. There is no cheating. Cheating is gaining an unfair advantage over someone else. Anyone can do this. It's so simple you could do it by accident. Exploits are not cheating. Superbouncing in Halo 2 is an exploit. Wavedashing in Smash Bros is an exploit. Reload animation canceling is an exploit.
I agree that Bioware should be aware of the problem, and something of this magnitude should be fixed. But if they ban a single player over something that they allowed them to do, then that is just wrong.

Why? Why are there so many ignorant people here that believe that animation canceling (reload canceling) is an exploit?
Why?
Did they miss the memo? They obviously have missed the official BioWare statements that explain that it is A FEATURE. NOT AN EXPLOIT.
