cerberus - some centurion I overlooked
geth - probably hunters and prime turrets, the others are easy to evade
reapers: I guess marauders, a bunch of them can get you down quickly, especially if a banshee or a ravager forces you to leave cover
What Kills you most?

Lag and my character taking cover in mid-sprint.
Those kill me the most. I wish they would remove the system where you take cover if you are sprintin.
Oh yeah and Geth Rocket troops are annoying.

Lag.
I used to die a lot due to «One Button To Rule Them All» too. But there's a solution if you're on PC so now my worst enemy is lag =)
And if you want the enemy it depends on class I play of course. But I do hate Pyros because of stupid inconsistence in range of attack and visible animation. And that's funny but basic troopers of all factions are the most common cause of death

I myself mostly.
Not to shower myself with praise, but I've come to be good enough to deal with every standard and the not-quite-standard situation you could run into the MP so far.
What's left is the situations that I myself manouvred my then soon to be butchered ass into, aside from the occasional bad luck like running from one grenade on top of another and *blamm*.
Which is a good thing, because I know I was at fault and I can learn from that, so I won't be as likely as to run into such a perril too soon again.
However I am bloodknight'y enough to simply not care for most of the time and just charge for ****s and giggles and the glory (I blame my Vanguard blood, even though I play all classes), so if you see me dead somewhere, there's about a 80% chance I just went there because I wanted to bludgeon that dude instead of merely shooting him (depending on playstyles then again, when I am ruinning Engineer, I'll be happy to zap them to death, but my Batarian Soldier is exactly the kind to melee Banshees (works mostly
).
The obvious other serial killer is of course bad luck and lag. I've been on the recieving end of the magnetic hands a full share aswell had matches with lag so severe I wouldn't know I hit the dude I was shooting at until three seconds later and dying by late-registered shots when in full cover, but those things have their own joys (did I tell you I play kind of bloodknight'y already?).

Playing too aggressive, steaming ahead and getting caught in crossfire is what kills me most. I hate camping and love to stay on the move. Occasionally I over reach my abilities but at least I'm having fun.

Husks. Definitely Husks.
For some reason, I never see them coming. Ever. And when they grab me, I am never alone or in a safe place. It's always while I'm standing on a grenade, or a brute is getting ready to charge me, or a Banshee is closing in on my location, or a Ravager has me in clear line of sight. I don't think I've actually been killed by a reaper unit in over a month now unless said unit killed me while a Husk was punching me in the face.

Rezzing people who died. I get killed doing this more than doing anything else in MP.

1. Lag
2. Tactical cloak bug
3. Stunlock
4. Husks (actually Marauders and Brutes get the killing blow...)
5. Phantoms (although often an Atlas/Nemesis/Centurion/Combat Engineer takes credit from me running from Phantoms)
6. (Trying to) revive fallen comrades.

I have high awerness so when I die I don't know by what
But seriously now
1. TC bug
2. failing reflex check on avoiding Prime lasers/Atlas rockets
3. random events, like running from one enemy straight over a grenade the moment it explodes or withdrawing from heavy fire just to bounce into a pyro who was simply wandering around minding his own business or should I say their business
4. unexpected banshee teleport and instakill
5. dat invisible turret :|

for my GI the deadliest "weapon" on the map is; swarns, and Ravager acid. Annoying as hell!

As others have said:
--Stun Lock: doesn't matter if it's the Phantom regular melee stunlocking the sh*t out of me; Hunters stealth-shotgunning me out of nowhere; or Primes stunlocking me from across the map (and usually around one or more obstacles) while I desperately try to get into cover.
--Lag: playing a Vanguard while I'm not hosting has become an exercise in F*CK YOUR EYES. Banshee sync kills combined with lag are somehow more annoying than the fact BW deliberately increased the range at which they can occur. And several times now, I've noticed when my Batarian does a heavy melee on a Husk, the Husk jumps on me to face-hump me at the end of the animation. That last one doesn't seem to happen with any other class.
--Anomalous glitches I cannot easily define: yesterday in one Gold Match on Goddess, every member on the team got one hitted by Assault Troopers. I literally took a single hit from an Assault Trooper and went from full health and shields to dead. Didn't know what the f*ck killed me until I looked at the kill counter. And then, I watched as every single player suffered the exact same fate over the course of the next few minutes. This didn't happen again the entire rest of the day, just that one match. The day before that, we encountered invulnerable Phantoms. I don't mean Phantoms with their glowy-hand powers engaged. I mean Phantoms trapped in Stasis. I'd shoot them in the head, with each shot connecting, only to watch as their barriers never dropped. Again, this happened in one match, and never happened again afterwards.

Ravagers because they do so much damage and start firing the minute you leave cover. Seriously, banshees aren't too bad as long as you keep your eye on them but ravagers stay out of sight and kill you before you know that they are even there.
