This is exactly it. No if, ands, or buts about it.
And even then, it's mostly rolling ONTO a grenade or sticking to a wall in a vain effort to run away from said grenade.

Ravagers have that nasty habit of positioning themselves in the strangest of locations, places where I fail to look. I'll move out into a lane to start murdering some Cannibals and Marauders, and BAM, there's a Ravager somewhere yonder, with a clear lane of fire on me.
Cerberus Engineer Turrets, on the other hand, are a different world of hurt all together. Those bastages stick around between rounds. I've had it where I push in to clear out a new area, and there's a turret placed conveniently on the side of the map, where nobody ever looks. And the worse part is, this happens BETWEEN rounds as well... At the end of a round, I'll run to get some ammo and/or setup at a strategic point in the map, and along the way, in some random corridor, there will be a turret set up, like it was waiting the entire round for me... and I end up being that idiot that gets killed between Round 7 and Round 8.

And two or three husks sneaking up on you is a death sentence. The first interupts my heavy melee to grab me. If I survive the marauders and ravages that WILL appear out of nowhere the second that happens, the next one (that was waiting patiently the whole time) grabs me before I can do anything at all! I'd prefer to be banshee magnet handed, then corpse camped by a phantom in a circle of turrets and pyros than have to deal with the husk conga line!

Overconfidence. On most builds I put points in fitness last.
Magnetic hands and invulnerable Phantoms (the only thing I'd rather BW would have nerfed)
Getting swarmed because some folks think they are ready for gold but are not.

Everyone else, I tend to get shot by cannibals when I am meleeing husks to death, or I roll out of enemy fire onto a grenade indicator with a split second fuse.

Fun fact: The ME3 endboss Marauder Shields was the only time on my Insanity Run where I was genuinely frustrated in my inability to proceed. He tried the very best to keep me away from the ending.
vs Geth: Geth Turrets, Stunlock and every now and then Hunters when I'm not Geth and my team's not paying attention.
vs Cerberus: Assault Troopers, along with the occasional cheating Atlas shooting across the map through cover.

For me this, combined with Super-glued covers, is by far the most common cause of duying.

A Husk hanging around your body after you die might as well be a Phantom. Can't count how many times I've seen someone use Medi-Gel, get jumped by a Husk as soon as they get up, and die all over again.

You'd think an invading army would have better things to do than stand in a circle around their dead enemies. I refuse to believe husks know about anything, let along medi-gel!

lack of communication. bunker down expecting allies to hold their spot because they have been for past several minutes and there is not reason to move.. they all leave the moment i start with one direction and i get shot in the back by something that should have been killed by my useless teammates.
RNG enemys. Banshees, Phantoms. Banshees are the worst kind of thing to add to any kind of game and playing reapers is just a boring chore most of the time due to them. same with Geth but at least they are not random in their BS. anything random is never worth it. Never.
