I dumped my previous install's saves into the Documents\\BioWare\\Mass Effect\\Saves folder and indeed, the saves are present in-game to load but not the achievements.
However, whenever I quit out, all achievements gained are lost and some settings revert to default.
Screen Res/Shadow Detail settings save, but not Motion Blue or Film Grain. Key bindings save, but not mouse settings (speed/reversal).
I've started a new game (not NG+) and still the same. I've checked the folders and everything is read/write.
Any ideas please?

NVM, profile issue, file was empty for some odd reason. Fixed.

I think I'm having the same issue, where all of my accomplishments disappeared. What exactly did you do to fix it?
