Yes, she really says in ME2 her father used his Y chromosome. But does not go much into details. Since human Y chromosome does not carry much genetic information, that sounded a bit odd to me, but I've just decided to ignore it. M.

Still, it is actually possible for women to be born with XY chromosones. It's called androgen insensitivity syndrome. Even if she had been born male-bodied, she clearly identifies as female. A transwoman is still a woman.

I agree, gender is a ternary in English…masculine, feminine, neuter.

Sex, on the other hand, is a spectrum.

The way I understood the scene, instead of using his X chromosome twice he modified the Y one turning it into an X one with traits he desired (I don´t know what the X chromosome actually codifies so no idea how much sense it makes, although ME seemed to stay away from LEGO genetics). I think if she was actually a XY she wouldn´t have developed fully as a woman, but we know she has ovaries, so she´s likely XX.

Yes, but Miranda was not born in the standard fashion, but rather engineered, making that irrelevant.
