I don't know if this is the right forum for this, and I suppose as my description of the glitch might have some spoilers for the previously mentioned priority, so reader beware? Anyway, on with the glitch...
Ashley has disappeared on me three times now -- twice in separate playthroughs (though with my same imported Shepard, I'll add) and once when I reloaded an old save to give it an extra shot. I get clear up to the end of the mission, shoot Udina, and greet Bailey. During the whole cutscene this most recent attempt I made sure to see if she was there the whole time, and she was. Then, Bailey starts ushering people into the elevator and all of a sudden I get a nice shot of the Citadel background. One of my squad members appears from the middle of the screen and walks offscreen, and then it's back to the plain background. It shows me this for a bit then fades to black, and the cutscene with Kai Leng talking to the Illusive Man shows. Then it's straight to Shepard in Commander Bailey's office. I never get an option to invite her back onto the Normandy, she's not marked anywhere on any of my maps, and it's like she just disappeared from the face of the planet. I don't believe any further mention was made of her at all during the rest of my playthrough.
Does anyone know if there's something I might've missed that might've caused the glitch? The first time I was pretty sure I missed some dialogue with her in the hospital, but I was fairly rigorous about checking for new scenes/mail this time around. I've got the Collector's Edition and I'm playing a FemShep, so no Ash-romance, if that makes any difference...I remember reading about someone who had a similar bug once, but for the life of me I can't find that random forum post that mentioned it. Any help anyone could give me with this would be immensely appreciated -- I want my Ash back!

Are you saying your not getting the cutscene when you return to the Normandy after baileys office?

Just enter the Normandy like usual it should go to a cutscene.

Nothing again, I went straight to talking to Anderson.
But! I reloaded a save from right before I went into the elevator during the mission and, although the weird cutscene glitch happened again, when I tried going back to the Normandy again, she was there! I'm so glad this was such a simple fix. Thank you so much!
