People who try to shoot bosses with the rocket who don't shoot at floor. Or when they use RL on bosses with like half of their armor (or eventually with full HEALTH, like phantom)

Not dying entire match and survivng very hard encounters with multiple phantoms/banshees/geth hunters, but dying to the weakest enemies at wave 11.

I just got tossed into a game during the extraction round, and my countdown was apparently boned since I thought I had a full minute to mosey over to the LZ when in fact I apparently had, like, 20 seconds. It's not exactly embarrassing, but I did feel like such a loser for being a level 20 who missed extraction on a bronze game (random/random/random) that I left the lobby as soon as it loaded without explaining the rng bullpucky.

Running up to cover (i know theres a turret on the otherside) to try and rez fallen teammate from behind cover - some how double taps X and instead of ducking into cover before turret kills me slide vaults over cover straight onto turret and dies in full view of other two teammates! <facepalm>

Haha, new embarrassing moment for me... Mistaken a walking Banshee to have lost her biotic jumps, thus tried to walk around her. Fatal mistake >_< Gosh that was a wtf moment for me

I had a brain fart yesterday, may have been the heat, took off in the wrong direction with the objective......probably took 45 seconds before I realized...

Thinking: Ah, time for pizza delivery and continuing blowing up things hard with my Geth grenadier. Some time into the wave I wonder why noone would pick up the pizza and suddenly realize it's a hacking objective and I have to fight through another horde to get to my mates.
Ah well, it's hunter mode tunnel vision I guess.

The other day I went into a bronze game to level my promoted infiltrators. I was about lvl 6 at this point, with my salarian infiltrator, trying out a light shotgun + assault rifle build for duration and power spamming.

So I drop into the game, run up to the two atlases with my evicerator and:
"...you don't get atlases two at a time on bronze. I must have been put in a late silver match. I'll just check the scor-GOLD!? Aww crap!"

After that disaster, I actually did quite well 'til the phantoms showed up on mass and we wiped. That build has become a favourite of mine since then.

cobra to the atlasses: passed on BOTH in between the legs...

Match starts however I had been playing my adept far too much and instead of statis that large group I charge right into it and die, 10 seconds after start of wave 1. I lolz'd, good one.  I have done the same.  Be wary of the Y button...it is the root of all evil.

The one that I'm regularly guilty of is forgetting to turn on Tech Armor/Fortification/Barrier etc. at the beginning of a match.

I play on xbox, Yesterday I was in party chat with my brother, He left and I didnt notice that I was no longer in party chat until I had a bit of a temper tantrum that would make a 5 year old proud, then heard "Dude calm down, its only a game"

Kroguard platinum on Glacier...

Fell asleep on wave 6 or 7... Woke up in the scoreboard screen with all red gamer tags.
In the lobby I immediately had 2 red X's next to my name.

I felt so bad I PM'd the host and apologized profusely.
Got a new friend out of the experience.

Plenty, but one today was pretty painful. Sprint towards wall cover, hit A- instead I totally miss cover, do a combat roll forward, and enjoy a Phantom sword to the gut on landing.

Biotic charge on a group of cannibals and then *brainfart* "where's that nova button?" Yep, happened more than once.

"Yeah! Gonna save thay guy, revive him! Me can be hero, just for one day!" Turns corner and there's a turret... Yep, more than once too...

The worse of all... Playing, everything is OK, room's silent and all of a sudden enters wife, the instakill indoctrinating banshee. I forget to unplug the mic and everibody can then hear me after being turned in to a tame pesky husk "yes, gonna do it now"... Game over.

I was killed by a random Swarmer and lost the match.

Another, I was playing a match yesterday and I hear a Missile Laucher, I look at the log and I see only one kill:
