There are a fair number of things that are bugged in the official campaigns. For example, at the Haven, the hot springs do not check the skills they say they do (they say survival, which would seem to be correct based on what you are doing, but something else is checked). Another example in the original campaign is a quest in Neverwinter where you are supposed (or spend, I forget exactly) to receive some gold for an action, but don't.
Someone did go through and fix these (I confirmed by the toolset the two above bugs exist), but they also broke many things as part of their effort to "fix", and applied their personal preferences. Unfortunately they didn't break up their work into quest bugfixes and personal preference, and the personal preference stuff breaks things terribly, it introduces many game breaking bugs where there were no game breakers previously. Because the work wasn't separated, and there isn't a full list of what was done, the work was essentially useless.
