   How was it that Harbinger missed Shepard when he hit everyone else dead on with his other beams?  Did he lose his contacts or something?

Harbinger intentionally missed because deep down Harby really loves Shepard and can't bring himself to destroy him/her. The new Synthesis ending will feature Shepard surviving and eloping with Harbinger while pretending to be dead because the galaxy can't understand their forbidden love.

This.  Harbinger has a poster of Shephard on the ceiling of his room.  Both female and male Shep.... somehow.
