Also two things to say:
1) I know it sounds unappreciative of their effort but another EC. The EC was dreadful imho.

Personally I would love to see a mission related to the Rachni - perhaps once the queen is set free they issue a request to aid them in re-settling their own homeworld before they will help with the crucible.
What would be even better would be a mission on a similar level to Tuchanka or Rannoch but on one of the homeworlds we haven't seen yet. A mission to aid the Elcor would be a great opportunity to see some of their living tanks in action, perhaps even to hear some Elcor Shakespeare (in one of the inhabited areas - perhaps an actor attempts to keep morale us among the frightened civilians by putting on an impromptu reading).
Perhaps (and this would be a bit of a proof-of-concept for the idea that synthetics will eventually turn on organics) we could have a completely standalone DLC that doesn't take place during the game at all (or works in a similar way to visiting the Geth consensus, so takes place within a simulation in a long forgotten archive) that shows us the many attempts by the catalyst at mediation, perhaps including the effects of the premature attempt at synthesis. The extended cut IMO really did improve on the original ending, even to the point of making the presented options much more palatable, so consolidating that by introducing a mission earlier in the story that foreshadows (and pre-emptively confirms) some of the catalyst's information would be quite interesting. Plus, not only would it lend the catalyst a more solid context within the story, but it would also give Shepard a reason to believe it - they would know it's not just an assumption, because they've seen it happen with their own eyes.
*Although, that said, there are still possibly Cerberus forces on Earth that may have orders to capture Shepard's body (either alive but pretty broken if Shepard survives, or dead if otherwise) as part of some contingency plan. A mission to beat Cerberus to it would allow both those players whose Shepard died AND those whose Shepard lived to have their reunion with their crew and LI, whether it be a happy event or a bittersweet one involving a funeral.

Agreed on all of those. Having a chance to finish off Vido in the Omega DLC if you let him go in Zaeed's loyalty mission would also be cool. I hope we get some new enemy types in the Omega DLC, too. Maybe a return from the Collectors, since I'm pretty sure it's implied at least once in game that the collectors are out there assisting the reapers in some places.
Actually, in -general- there are some enemy types that were prevalent in the first two games that I'd like to see reemerge here and there. The mechs from 2 were cool, for one thing, but they were totally gone in ME3. I realize we've got the ridable mecha suits now, but that doesn't have to mean the giant robots are just -gone-. Hell, having you fight some of those guys -in- one of the cerberus mechs would be pretty awesome.
On the backstory one, also, It always bugged me that ME2 and 3 had no quests that related to those things. Especially given the nature of the third game, it was really kind of annoying that Shep -never- tries to contact his/her parents, or any old friends from earth, or anything like that. I -really- would have liked to have an N7 mission or something for each background/origin. Imagine the awesome if they used the family-tech from DAII to give the spacer sheps parents that resemble him/her. Fighting side by side with "mom" could be seriously cool.
Kal Reegar deserves better than a one-line "killed in action" report. It'd be nice to at least -see- his heroic sacrifice.
Visiting the crucible might be neat, especially if the content were designed to support the sense I had of "What -is- this thing, and do I really want to be building it?"
Personally, I'd like to see a patch that makes Kelly Chambers, y'know...look like Kelly Chambers, instead of "random human NPC".

You realize there are fans on these boards who have specifically asked for something like the refusal ending, right? Just because it wasn't what you wanted doesn't mean they're ridiculing you by including it.
Nobody asked for that. All the threads/fan fics/whatever that talked about refusal specifically centered around the idea where if your EMS is very high you would be able to pull through and defeat the Reapers. What we got was:
Catalyst: Choose.
Shep: No.
Harbinger: **** you.
Everyone dies, reapers win, game over.
That's the opposite of what people wanted in a refusal ending. That's like asking for sandwich and getting a literal **** sandwich; not exactly the same thing.

Also would like to see a little minor thing added with the omega dlc to find out what happened to Vido Santiago if he survived in ME 2. Or is that already handled in a quest on the citadel which i missed?
And a trip to Palavan would be great too, would like to see how the Turian homeworld is.
And a mission with Hanar or Elcor or Volus. Preferably a dlc which handles all three seperately, much unexplored lore to be found in a big mission with them and their culture. And by mission I mean to one of their colonies or perhaps homeworld. Would really like to see that.

1)- the "breath" scene rescue missions (we know its shepard, bioware said it was)
2)- dark energy DLC ( a HUGE PLOT from ME1 and ME2 and was NOT in ME3)
3)- retake omega
4)- CURE Thane, recover legion and save mordin
5)- CUPCAKE DELIVERY mission (sorry but it HAS to be in there)
6)- happy ending DLC
these are just some ideas i thought about in 10 seconds but put ALL those ideas into a single player DLC and POW make tons of cash and you make EVERYONE happy

How about a DLC that is a series of missions for the 'honorary N7s'. It could utilise randomly generated characters if you avoid the multi-player, or it could let you choose up to three of your promoted multi-player characters (filling in with randoms if you don't have three available). This could easily take place within the time-line of the main game, and it could even incorporate some of the ideas I mentioned earlier (such as visiting the Elcor, Hanar, Volus, or even perhaps Vorcha homeworlds) without having to worry about significant consequences or new squadmates during Shepard's story.
