But does it matter, or can I just play from my auto-save where Shep wakes up then I decide what to do and still see all the new stuff?
I know the official stance is "replay from start of point of no return"

No. The Priority: Earth mission beforehand has been altered. Start at the "point of no return" or the EC won't kick in.
