Cailan most definitely had his torso crushed by an Orge, and we saw Duncan kneeling over Cailan's corpse before his face was introduced to the business end of a wicked 2-handed axe. I think short of coming back as zombies, Duncan and Cailan's roles in the story ended at Ostagar.

Whoever did that picture should die IMMEDIATELY!
But from what I've read, I am pretty sure it's Dog. With that "another chance to recruit Dog" thing... Heck, even if not, he escapes the cage while being ill, avoids trillions of darkspawn and somehow passes the player on the imperial highway, only to then be blocked by a few darkspawn, to then return back to seek help, and finally he finds the player... Quite a story.

I hope it is a certain Ash Warrior for a revengeful purposes. I failed a pick pocket attempt on the Ash Warrior leader and then later as I neared Duncan I overheard him telling Duncan I was spotted with my hands in somebody's pockets put Duncan stood up for me. Made me so proud that I followed that tattle-tail and then picked pocked him for a minor revenge.
