I got near the end of NWN2 where I must defeat Black Garius
in the Inner Sanctum (I used the True Names Scroll at the battle in the Keep).
I killed all his allies and when I'm trying to kill him he keeps regenerating
his health over and over via the portal. Is there anything else I need to do?
I read a walktrough of this fight and it didn't mention anything special that the
player needs to do. Could this be a bug?

Sorry, the Keep bug is so common I thought of it immediately.
Your problem seems to be a bit more creative.*
First, did you get a prolonged conversation with BG and all your companions before the fighting started?
Second - and probably the first thing you should try - can you reload in the room where you just joined up with everyone and get it to work right?
Third - when you first arrived in the Vale, did you get a conversation with everyone, noticing that Neeshka was gone and did it all go correctly?
Fourth - are you fully patched? And you're not using that silly "Plot Fixes" mod by WRFan, are you? That thing seems to cause as many problems as it solves.
Fifth - you didn't somehow cut off the cutscene and force the action, did you? In general, you have to let the cutscenes play even though the enemy then gets the jump on you. Consider it the price of being a hero.

Yes, but I skipped most of it since it was very long.

I worked really hard on this fight, I feel very reluctant to do it all over again. What are
the chances that this time it will work?

I ordered the game from Steam a month ago, so I suggest that all the necessary patches all
already in. But I really don't know which patches are included.

MotB is 1% as buggy as the OC. just judging by the number of times game breakers have been reported here over the last 2 yr. ( the code itself also looks better internally )
no doubt, this is part of the reason it gets generally very high kudos.

MOTB has one huge, glaring BUG - that isn't necessarily apparent.
There is an intended encounter that never happens. I will be careful about posting any spoilers here, but I can detail it elsewhere if you want to look into it.
