Since the release of the EC I've noticed a surprising similarity between how the Catalyst's logic functions and the logic of the Qun.
"Karasten are soldiers. The Qun made it so. They can never stray from this assigned path, never be other than they are meant to be. But they are free to choose within that role. To accept and succeed and deny and die."
Shepard similarly has a role to fill. The Catalyst is the one responsible for bringing Shepard up to it's chamber and presenting the three options. Shepard is free to choose within his/her assigned role or refuse and die along with the rest of the cycle.
I can only conclude that the Qunari are responsible for building the Catalyst and possilby became the first Reapers. Mystery solved.

The Arishok wasn't the only Qunari. Their logic survived to screw us over in the future.
