Recently the game has stopped giving me the "Resume" option in the start menu - I have to manually load my last played character & savegame instead.
Also, the game puts settings in the options screen back to the defaults, I think. Axis inversion on the controller, subtitles, helmet settings, squad power use are all on different settings than they were when I last played.

And I've lost the bonus powers. None of them show up anymore in the Med Bay.

Achievements are actually in a weird half-way state. They're still there on my Xbox profile, but sometimes I get notifications that I just got them. It's weird.
And all of the things mentioned in my original post happened two or three times.
