[Clayless]

This is only minor but still enough to take you out of the moment as it's the one thing that catches your eye, but when the new character cutscenes (the flashes before your eyes and remembering the dead ones) appear at the ending some of them don't fit in the screen right and have a thin red line going across the top. Seeing as though the scene is in muted grey colours the thin, but bright, red line grabs your eye and takes your attention away from the scene.

So far the only one I can remember this happening in is Vega's one, though I'm sure it happens in Mordin's too and some others.
