The same thing used to happen in ME1.
I remember on my Insanity run (and It's fresh, I played the entire trilogy this year) on Feros.
Back to the camp, the last part before going underground... I happened to smash a husk behind one of the wall using special bullets. It wouldn't come out after a while. I had to play again the entire mission because I forgot to save.

lol. I just broke my 4 million drought this past weekend. Not going to complain about the three I did manage to get, but I really wouldn't mind an Indra upgrade or two at this point...it's like the game just never wants me to snipe.
