But you don't get the VS on the ship till a good way into the game. And even then you can refuse them.

Exactly. Ash or Kaidan was probably the XO prior to leaving Earth, but after that, they aren't even assigned to the Normandy. I believe that the VS has to request permission to board the Normandy after you take back the Citadel from Cerberus.

In ME2 it cannot be Miranda because she can leave the ship when Shepard was not on board. Pressley had to stay on board when shepard was not on the ship

While I am a great fan of Liara, she is not an aliance officer and is more a specialist (sciences).
In ME 2, the vessel is a private vessel and does not follow Military protocols, so it goes without saying (although it is, in fact, stated at one point) that Miranda is the Executive Officer (after rescuing Jack) at least for all intents and purposes in ME 2.

If the VS isn't around I guess it is either Joker or Adams, depending on which lieutenant has more time in grade.

It's whoever you want it to be. Why? Because it is not stated, so you're free to decide who you want it to be. Personally, I'd go with Adams. He's alliance with a long service record.
Part of the problem is the free mixing of ranks between ground and naval designations. Take Kaiden for example. Lt in ME1, "Staff Commander" in ME2, commander being a naval designation. Major in ME3, which is equivalent to a Lt. Commander in the navy. If Adams is a naval lieutenant, that's the equivalent of a Marine Captain.
